How do I enroll in my insurance benefits?

In order to enroll for your insurance benefits the Fund office will need a completed enrollment form. Your employer will provide this enrollment form, or you can call the Fund Office and one will be mailed to you.

How do I enroll my spouse/child under my insurance benefits?

The Fund office requires a marriage certificate to enroll your spouse & Birth certificates to enroll your children. These certificates can be mailed to the Fund Office or sent via the Viveka Health app.

What are the eligibility requirements to receive a pension?

If you have earned 5 years of vesting service after January 1, 1999 and are 55 years old, you are eligible to receive a vested pension. Prior to 1999, you must have earned 10 years of vesting service. If your benefit is governed by the Rehabilitation Plan and your benefits are determined under the Standard Plus Schedule: and you have 20 or more pension credits at age 60 while you were in covered employment, you are eligible to receive a regular pension; or before January 1, 2009, you have 20 or more pension credits at age 45 while you were…

How long is the application process?

It can take up to 90 days to process a pension application. In some cases, the pension application process can take up to 180 days.

What documents do I need to submit with my pension application?

You must submit a completed pension application along with the following documents: original or certified copy of your birth certificate an original or certified copy of your spouse’s birth certificate and marriage certificate (if married) an original or certified death certificate (if widowed) divorce decree (if either you or your spouse were previously divorced)

Can I get a lump sum payout? Or rollover my pension benefit?

To be eligible for a rollover distribution your benefit must be paid in the form of a lump sum. The Plan allows for a one-time lump sum distribution if the actuarial present value of the benefit payable over your lifetime is less than $5,000.00.

How do I get a pension application?

You can print a pension application here or call the fund office at 773-685-0340. Please note, pension applications cannot be faxed or emailed. Pension applications must be delivered in person to the fund office or mailed with the required original documents enclosed. If mailed, required original documents will be returned to the sender’s address.

Who is Eligible for Legal Assistance Benefits?

As an employee in a collective bargaining unit of a contributing employer, you become a participant in the plan on the first day of your fourth month of employment. To be eligible for educational assistance benefits, you must be a participant for at least two months before the start of the school year. For example, if you began working on January 9th, your eligibility for benefits would begin on April 1st. How the Legal Assistance Plan Works

The Plan provides free legal assistance through a CTM Legal Group for certain legal matters. You and your dependents can each receive up to 45 hours of free legal assistance each calendar year for these covered matters. You and your dependents can also receive unlimited hours of legal assistance at a discounted rate for other legal matters. Additionally, you and your dependents can receive a special fee reduction for matters involving a personal injury and related property damages.
