When am I eligible for insurance benefits?

As an employee in a Teamsters Local Union No. 727 collective bargaining unit of a contributing employer, you become eligible for benefits on the first day of the third month following the month in which you begin covered employment. For example, if you begin covered employment in January, you become eligible for benefits on April 1, the first day of covered employment.
If you are a Movie Tradeshow Division Participant, you may become eligible if your employer makes contributions to the Fund on your n=behalf for working at least either (i) 170 hours in a one-month or consecutive two-month look-back period or 1,100 hours in the immediately preceding 12-month look-back period. Your coverage will begin with the second month following the end of the applicable look-back period.

How do I enroll in my insurance benefits?

In order to enroll for your insurance benefits the Fund office will need a completed enrollment form. Your employer will provide this enrollment form, or you can call the Fund Office and one will be mailed to you.

How do I enroll my spouse/child under my insurance benefits?

The Fund office requires a marriage certificate to enroll your spouse & Birth certificates to enroll your children. These certificates can be mailed to the Fund Office or sent via the Viveka Health app.
