You must submit a completed pension application along with the following documents:

• original or certified copy of your birth certificate
• an original or certified copy of your spouse’s birth certificate and marriage certificate (if married)
• an original or certified death certificate (if widowed)
• divorce decree (if either you or your spouse were previously divorced)
